Nowhere is that more important than in the area of finances. This Financial Agreement shows our respect for your right to be informed in advance what our expectations are when it comes to finances. Should you have any questions or concerns, please call (516) 268-5814 or (516) 268-5997 and our dedicated staff will be happy to answer them.
Dental Insurance:
We will gladly file your dental insurance claims and help you in obtaining the maximum dental insurance benefits available for your policy.
We accept all major credit cards, cash, money orders and personal checks. Our practice also participates with CareCredit which assists patients who need a payment plan (up to 12 months, interest free) to get the dental care they require. Please make arrangements for your CareCredit Health Card before your first appointment.
